*The explanation below contains details about the creation and use of Patient Reported Outcome Measures (PROMs). If you have any questions on this explanation or want to know what your results mean, please contact your clinician.

EQ-5D Tool

The EQ-5D is a simple health survey created by a group called EuroQol. It helps people describe how they feel in different areas of their health. For over 25 years, people around the world have used this survey during doctor visits, in research, and health studies. It is available in many languages and only takes a few minutes to finish.

The survey has two parts. First, there are five questions about how you feel in different areas: moving around, taking care of yourself, doing everyday activities, pain, and feeling worried or sad. You choose from five answers for each, ranging from no problems to extreme problems. When you finish this survey, you will see your score for each section. A score of 1 is the best, and 5 is the worst result. You will also be able to see your previous surveys. So can your doctor. This helps to see how things are going for you (quality of life) over time and check if treatment is working.

The second part is a scale where you rate your health from 0 (worst health) to 100 (best health).

In the registries this survey is used for any condition. Doctors and patients can both fill it out, and it is usually done once a year.
